Audio files in osu! are made up of several different types.
These are .ogg, .wav and .mp4.
Maybe there is more but I don't know about them. One failsound file can be .ogg and the other failsound file can be .wav. And you may not have noticed this.
So when you die the osu may have both started at the same time.
So delete any failsound files with the .ogg or .wav extension you don't want.
And if you can't fix the problem, upload the skin to MEGA or Google Drive and submit it here. I will download it and try to fix the problem.

You may have confused things a bit. There might be failsound and pause-loop playing at the same time.
failsound is played when you die.
pause loop is played in pause AND fail screen.
