Insane:
For starters, OD 5 is way, *way* too low for an insane. It should never be anything lower than OD7, and for this map I would highly recommend OD8 given that you're doing both a low AR and a low BPM
In the intro, you have a tendency to start new combos way too often which I don't agree with because it hurts readability, but is ultimately sort of harmless. But after that, it starts to cause timing issues. In general, save new combos for the starts of measures or only if it's really obvious that it should be a new combo. So for insane, I'm going to go ahead and list all of these, I'll leave it up to you to find the ones in the other difficulties.

00:00:748 (1,2,1,2,1,2,3,4,1,2,3,4) - way too fast for the song's intro IMO, and plays really awkwardly. Cut them to 1/1 notes and increase their spacing to keep up intensity
00:08:856 - There's a really bad over lap here with the end of the slider from 00:07:437 (1). Move the circle so it overlaps perfectly with the slider end.
00:09:869 - Move the (2,3) from 00:10:275 up a couple points and move one into their place. This flows nicely and gives the added benefit of putting the slider on the new measure, justifying it as a new combo. But again, watch the overlap with that first slider from earlier.
00:13:315 (5) - again, bad overlap with 00:11:896 (2,3,4).
00:20:410 (1,2) - Listen to the drums in the background. Those are big hits that deserve big jumps!
00:23:450 (1) - this is a huge cop out. I want to play all those synth notes, not just follow a slow rolling slider.
00:26:693 (2,3,4,5,1) - On the "follow the synths" thing, just in general for this section try and map sliders to the synth holds, the 1/1 slider at 00:27:302 (5) really feels out of place. (and again, 00:27:910 (1) - should not be the new combo)
00:46:761 (4,5,6) - Having this as a triple doesn't seem terribly justified. There's nothing in the instrumentals to match it, and feels kinda silly.
00:57:504 (1,2,3,4,5,1,1,2,3,4,5,1) - The same problem I mentioned with the intro, but that's ultimately your decision.
00:59:126 (2,3) - if you want to keep these sliders here, space them. They feel sort of clunky imo.
01:01:356 (3) - bad overlap with the end of the slider at 01:00:545 (4)
01:02:775 (3,4) - Bad overlap
01:13:923 - Listen for the drums here-- there's an awesome slot for a triple and you totally ignored it.
01:16:761 (1,2) - Here we have a special case Here, I highly recommend reversing these two (simply ctrl + g), and then setting the NC back onto the new measure. So we go from this:

01:17:775 (1,2 ) - bad blanket. There are probably more of these interspersed throughout the map that I recommend you go back and look for, but in a nutshell you can use the approach circles as guidelines to make your slider fit around the approaching note nicely.
01:24:869 (1,2,3) - Reverse the slider, then remove the single notes back on the top of the slider.
If you opt to do the above, also consider then reversing 01:25:477 (1,2) to make some nice triangles.
