Disable the widescreen support in all diffs, is needless since you don't have sb.
The artist is okay? I mean: http://ja.wikipedia.org/wiki/Kanon%C3%97kanon & http://tinyurl.com/lasl79z. According with the links the artist is: kanon×kanon and yes, the "×" is a unicode word and don't works on osu!, then the artist should be kanon*kanon: the "*" works like the unicode "×" https://osu.ppy.sh/s/25042. The same thing with others titles/artist with stars, hearts or some unicode word on them.

Hmm according with the link provided by Dake (from the Shiki Original Soundtrack) is just (TV size), I think is best use the title from the original soundtrack instead of the anime web.
