01:45:001 (1,2,3,4) - With this, the pattern would fit musically better if you kept from using 01:46:144 (3,4,5,6) - this pattern until 01:47:287 -
01:47:572 (2) - Hm, maybe position this ctrl + h'd from the end of 01:47:286 (1) - Flows better to me.
02:26:001 (1,2,3,4) - Just use even spacing here.

In general, I think the Insane still needs more work. There's still a lot of strange hard-angled flow that doesn't feel like it's representing the music very well and feels like it's just trying to be hard instead. See if you can get a few more mods mainly on the Insane and call me back please.

04:39:429 (3,4,5,6) - This whole part, impossible.
05:02:247 (5,6) - Unfitting jump.

Did some IRC modding concerning missed mods on the Easy and these points on the Insane.

00:30:858 (3) - This jump right here.. is really bugging me. It's just out of nowhere to keep your pattern pretty, but it plays rather poorly.
00:48:429 (1,2) - This jump doesn't make sense when you compare the same part of the music with 00:46:144 (1,2,3,4) -
00:57:429 (5) - Stack this with 00:57:858 (2) -
01:50:715 (4,5,6,7,8,9) - These returns to stacks are a little confusing to read. Think you can make it more clear?
02:02:715 (1,2,3,4) - This doesn't read very well afte doing a circular pattern at 02:01:858 (1,2,3,4,1) - The jump from 1 -> 2 is the culprit here. Maybe using more even spacing for all 4 instead of this random jump will fair better.
02:07:715 (5) - Change the spacing on this one too, considering the rest of the pattern.
02:59:715 (1) - This repeat arrow is slightly obscured by the hitburst from 02:58:858 (2) - . Should be easy to adjust.
03:35:144 (1) - I find this plays better unstacked

To be honest @ KDS and BAT's, I'm not sure about this break in the Taiko diff. at the beginning.
It has no reason to have such a - long - break, either I don't feel like it fits here in any way well. This isn't such a small issue. It feels akward, like it has been just cut at this part after this k-stream.

Please give me a reason why you used it here, else this should be unranked and mapped because of following guideline:
Breaks
All break sections should be artificially removed from the .osu file.
This also means to have in general no break sections as long as it dosen't fit.

Edit: I noticed right now, there are even more breaks. Why so KDS?
03:07:715~03:28:287 - those are 21 seconds are in general a not allowed time range, especially when the song it self is going on.
