Started playing God Hand yesterday. I'm currently at stage 2 (Normal), and I haven't cleared a single area so far without dying at least once. It's not the hardest game I've ever played, but it's definitely another one of those difficult-yet-fair games a la Super Meat Boy.

I've been very tempted to fire up God Hand again, since I never got far. I do remember it kicking my ass though.
I remember the controls being really horrible.

Bucky O'Hare on NES in hard mode when every single bullet kills you. I managed to get to the final boss on the real hardware though

Ignoring all the NES games I can think of, probably Super Meat Boy. It turned out to be one of my favorite Steam games though so I eventually got the Golden God achi. Honorable mention to IWBTG. It's harder than SMB, but in a very unfair manner, so it's more trial-and-error rather than being good at the game. At least the beginning, never bothered to go beyond it.
Ninja Gaiden: Black & Shin Megami Tensei: Nocturne.
IWBTG's difficulty is largely fake though; ie once you learn all the Gotchas you can zip through it. I found Mega Man 9 to be the same way, which was why I was glad Mega Man 10's difficulty was a bit more legitimate.

Shin Megami Tensei III: Nocturne on Hard mode. INSANE BOSSES.
Mushihime-sama Futari on ULTRA difficulty. It just makes Touhou's lunatic seem to be a piece of cake.
And IWBTG along with its Touhou crossover ("I Wanna Be The Shrine Maiden") are both quite difficult too...
I'd also mention Kagetsu Tohya - clearing it without using a walkthrough is not really a far cry from a slow-paced walk around the Ninth Circle of Hell.

My friend and me spent hours upon hours trying to beat it. Getting stars was nigh impossible cause you get like 1 a fight, and if you wanna transfer cards in, you had to spend like 5 billion.
i beated it with gameshark, otherwise it's impossible imo

For me it's currently The adventure of Link. My mom knows where everything are on that game and she's really amazing at it.. But I'm pretty much worthless on it... She even had to help to the last boss which I took myself XD
Gonna add beatmaniaIIDX (Which I know was brought up in an earlier post) to my list, because the learning curve is the steepest I've ever seen for a rhythm game. The jump in difficulty between normal and hyper is ridiculous 
I suppose in general bemani games are fairly challenging, but I find Jubeat very fun (and pretty easy). Never tried DrumMania or any of those though.

Metroid on the NES. I beat it, but the game was so poorly designed that to play it without any outside source would take a lifetime to beat.
A lot of NES games were like this. You had to basically bomb random walls in the original Zelda if you wanted to find anything.eat my soup wrote:

1. Most random bombing was optional. Neither the potion nor container was NEEDED to win, just made it easier.
2. Necessary bombing was most of the time deducted from logic by using the map and seeing there is no passage way into the room nearby on all sides. Plus, it tended to be in the center of the wall.
Metroid was like telling me to find a needle in a haystack over one hundred different times. Thanksfully bombing random places didnt take some form of money like in LoZ.

good point, I'd say stepmania was the hardest for me, I never was able to do 2 of the beatmaps I've downloaded with massive bpm changes (when at my best long ago)
If I'd try it right now... I'd fail worse than before, those maps are more terrible than 1 fingered 200bpm (and I use right hand only for stepmania) (how the hell was I able to keep up with it)

Dustforce is pretty damn tough towards the last unlocks.

This will sound stupid but honestly it's Touhou : Embodiment of Scarlet Devil because Bullet Hell ( Danmaku ) games are by far my least played genre D: could barely get past 3 levels on EASY
Definitely Resonance of Fate, until you get used to the extremely complex combat system, ur kinda SoL
Hmm... I would have to say either Deathsmiles, Triggerheart Exelica, or Otomedius Excellent. Just don't remember enemy positions for some stages. Especially on higher difficulties...

Hard to say, there are quite some games that feel impossibly hard. Just from the top of my head, I'd say Ghouls and Ghosts. I'll never understand how such a game, that's made of pure frustration, could have ever become so popular.
Now that I really think about it. It has to be Ace Combat 6 on Ace difficulty (Landing repair is disabled, not to mention that the enemies are more stronger and more accurate) or Ace Combat: Assault Horizon on Ace (Health regeneration is disabled, not to mention that the enemies are deadly accurate).

Don't worry, I've only ever gotten as far as the fifth level I think. Still had fun with it though.
for me is golden sun, megaman battle network 1 & 2 and age of empire... 
Prime was...meh...in the difficulty department. MP2 was really hard just because the two dimensions thing was ridiculous to figure out. But MP3 on Hypermode difficulty (yay Trilogyvizzy wrote:
