If I had no choice about using a mouse with any more than that (meaning it includes forward and back buttons on the side) which I owned, I'd destroy it and make dad buy a new one.

Right click, left click, and wheel. But if I turn it over there is a button to open up the battery case so that's 4.

For me, it varies depending on what mouse I'm using at the moment. Goes up to 5, usually 3 though.

I've got: Left click, Right click, Mouse wheel, 2 buttons on the left side and 2 buttons on the right side.
Though I just use the first 3 buttons, I still gotz loads of buttonz, so I win!
