Remember that these are all suggestions and that you can ignore any of them. Major issues are in bold.

General
Consider adding a Hard diff at least, the mapset would be way better.
Your mp3 is 320 kbps which is considered unrankable.
Remove countdown. It barely has enough time to be displayed.

Insane
Circle size +1 ? Right now it feels way too small. Also OD-1.
00:07:796 (1,1,1,1) - You don't need all these new combos nor these jumps (I'd see anti-jumps rather than jumps here).
00:32:894 (1,2,3) - These jumps are too much imo, reduce them a bit ?
00:41:913 (1,2,3,4,5) - ^
00:47:599 (4,1,4,1,4,1) - These jumps are too unexpected, it just makes this part confusing.

I suggest to increase the volume at least 10-20%. I can't hear any hitsound while playing!!
the "-" in Zero Zero Nine-One would not work when search, delete it.
3 combos color in Normal, 4 in Insane?... Make the combo color same?

Since you have 00:54:591 (2) with finish, why don't you add more?

Hum... the sound of the mp3 is a bit low. Amplify the sound by 200%~250% will be better. If you want, I can do it for you.
I think the kiai time is too long. I sugest you to end it at 01:00:091.
