"Extra" has, as far as I know, only been used in osu! maps on difficulties that are considered to be "beyond insane" and are often even featured alongside Insane difficulties. Granted, most of those are Touhou, but the reason it works is because "Extra" is featured as a difficulty alongside E/N/H/I levels, hence it makes sense in regard to osu! as a 5th difficulty name, unlike Normal/Hyper/Another or Basic/Advanced/Extreme, which doesn't really translate well into the idea of osu! having 5 difficulty levels. Ironically, Extra is easier than Lunatic and usually even Hard in touhou games.
