There can be two maps that use the same background. I think each map's background is an elegant way to show which map the player is referring to, acting much like an avatar. As such, it's preferable for maps to have different backgrounds to tell Title A, map A apart from Title A, map B in a quick manner. Backgrounds still can be similar, i.e. changing hues like in the Dragonforce maps would provide a way to tell them apart, but it should be enough to tell them apart unlike Revolution Deathsquad and My Spirit Will Go On.
There are also maps which use same background but of different gamemodes. I quick example of such is Defenders (taiko and standard) and Revolution Deathsquad (taiko and standard). I think this guideline shouldn't apply when comparing maps of different gamemodes because the community of the gamemode would typically know which map the background is associated with in that regard.
The terms "each map is to have a unique background" can mean that every difficulty in a map should have a unique background, so let me clarify. This guideline should apply when comparing individual difficulties in maps which have non-same backgrounds to other maps or difficulties and when comparing maps with same backgrounds as a whole to other maps or difficulties.
I would suggest this to be a rule, but it's unrealistic to search through every single map to find whether the background has been used. The guideline would be the following:

- Try to avoid using background(s) another map of the same gamemode uses. Having unique background(s) allows players to identify maps apart and provides an unique iconic representation of the maps. Changing your map's background hue or applying other edits is a good way to decrease the chances that the background of another map is the same.
