How can I get the (menu button background) file to display a map thumbnail?

You need to be an osu!supporter to have the menu background. Once you have the supporter tag you can drag an image file into osu! while you're on the osu! menu

Make sure you didnt disable it in the settings and that you are using Version: 2.2 or higher in your skin.ini

You open the skin.ini and look for the line saying Version:
Replace whatever its value is with 2.5
