Ok, first off, timing is not quite right. The song is not a 176 bpm map, but an 88 bpm map, but that's not very important here. Mainly, your offset should be 6,739. Your current offset is late by more than 10ms.
Also, get yourself an mp3 without that fade-out at the end, or make the fade-out happen later. This is obnoxious as you've mapped it.
Normal:
I'd suggest changing up the start. On notes like 00:12:205 (1) - there should be a note in the middle, whereas on notes like 00:14:932 (1,2) - these should be 1 slider. Also, some slight blanketting fixes are necessary, won't point them all out, but f.ex. 00:06:751 (1,2) - 00:18:341 (2,3) - or 00:24:137 (3,4) - .
There's also some alignment issues as in f.ex. 00:36:069 (4,1) - being the clearest in my eyes. A lot of the curved sliders + single hitcircle are also not well aligned.
00:47:660 - Here you should add a note.
The drop is correctly mapped. Too many angle sliders and horizontal/vertical 90° movements, but it's ok ^^
Hard:
Beginning, same as in the Normal diff. Some long sliders should be short and vice-versa, but blanketting is good.
In the later part you're missing some ternary rhythms like 00:33:341 (2,3) - here. I don't think mapping doubles in 1/3 is troublesome at all (like here 00:38:682 (2,3,4,5,6) - dunno why you didn't do all like this)
00:47:660 - add a note here
Now the drop is not good on this diff, SV goes way too high way too quickly for the difficulty level, and this makes it very weird for a Hard. I'd remap it with slower sliders and small jumps.
Insane:
Now I do agree with BounceBabe that this diff is way overmapped on the Insane diff, but that doesn't mean (in my humble opinion) that an insane wouldn't fit. You should abuse 01:09:478 (1,2,3,4,5,6,7,8,9,1,2,3,4,5,6,7,8,1) - to do your bigger jumps and also use the ternary doubles from earlier to add difficulty (as mentionned in the Hard, they're not hard to play, but with some spacing they'd make it more spicy).
