Top players mainly play other games of this type (VSRGs) because of 3 factors:
-osu!mania has some really bad mechanics, is very spammable, has weird scoring etc.
-osu!mania has next to no very hard maps ranked, so they get bored easily
-they are often just more comfortable with other games and don't really bother with adjusting to o!m

So, slight context. I have been playing rhythm games pretty actively since 2010, lurking many communities and being active in a few. My VSRG knowledge is mainly limited to 4K, but I feel that what I can say about osu!mania vs StepMania is similar. I've been trying to transition into osu!mania for a while and I've been doing somewhat poorly, at least compared to my peers (Halogen- and Etienne come to mind). I feel that my scores are only 80% of what I can do, but it's partially because I haven't invested enough time into the game to transfer a little more of my skill into the game.

Anyway, here's a couple of reasons why there aren't many top-tier players that start with osu!mania or solely play osu!mania:
- Simply put, map selection. Not counting SM converts (which are still limited for what it's worth, most converts only help with dense jumpjacking and jumpstream for the most part), there's simply no variety in terms of difficulty or pattern variety. The hardest ranked 4K maps are all luckshots in some form or another except for AiAe, and the super hard unranked maps are usually sparse, horribly done and even as pattern practice they are atrocious. You need maps at a certain difficulty level to actually improve, because you won't be able to improve speed/stamina if you're not pushing yourself. In addition, the charts must be hard enough for a long enough period of time for your speed/stamina to actually be challenged. That's why something like Bangin' Burst won't make you improve. You can use SM converts to improve, but again there's not enough variation to cover all aspects of speed/stamina like stream speed, jumpstream speed, etc. You can always convert maps from SM and convert them on higher rates even, but by then why are you even playing osu!mania? You can play StepMania instead. Leads to another point though, the only speed modifiers on o!m are HalfTime and DoubleTime. This goes hand in hand with the lack of variety of charts. SM doesn't have charts catering to every player, but you can play anything from 0.5 to 2.0 (with multiples of 0.1).

I'm also an experienced VSRG player. I've been playing since ~2002, and have played both Stepmania and O2Jam before osu!mania. I've been playing osu!mania since day 1 of the public release as well, so I've seen just how the community has changed and grown over time.

The same things Shoegazer said apply to 7k/8k as well. Both O2Jam (7k) and LR2 (8k) have heaps of absurdly difficult charts which offer a variety of different kinds of patterns for top tier players to push their skill ceiling with.

O2Jam has a wide variety of charts involving patterns such as nearly unpassable mashes, teleport notes, inverted charts (all LN), etc. These patterns are all effectively unrankable for various reasons in o!m, which means few mappers are willing to invest the time into making charts of similar difficulty, and the majority of players will never see these maps.

Similarly LR2 has a huge selection of insanely difficult charts with a wide variety of patterns, although far less emphasis on LNs than O2Jam. There is however an entire difficulty scale dedicated to LNs, and there are LN Dans, but the majorty of difficult LR2 charts have few or no LNs. Of course, just because they don't have LNs doesn't make them easy. I'd say many or most extremely difficult LR2 charts would also be unrankable in o!m (although they'd have a better chance than O2 charts).

Because of this, and other issues with community management, the entire ranking process as a whole, and the issues with o!m's mechanics, many experienced players don't see osu!mania as a desirable game to invest time in.
