You need to really define "hacking" because as a computer science major I really have a hard time believing a single person finds another single persons account info out of millions of players. If your getting scammed into giving your info out that's a different story and often times it's the victims fault for being dumb enough to fall for it. I've had a friend lose his league account and by the time he got it back the scammer used up all his RP and deleted all his runes with the rune combiner. He got some of it back via support (RP), but not everything (runes). Then his account got hijacked again because riot thinks it's okay to not verify password changes. As far as I know, you can counter Lee Sin with Udyr, Renekton, Pantheon and Elise, some people say that with Trundle too, but I didn't try it.
About Lissandra that champ is broken but I guess mid AP with mobility can counter her, and with AD ones, dunno.
And yes, that unwritten rule exists though on normal games trolls tend to rage and pick whatever they want sometimes xD

Lee Sin is terrible late game. Since you're talking about twisted treeline I'm just going to assume that he is on the opposing team and jungling. The best way to counter this is to play push champions. Pushing champions will help with being not ganked and more map presence. Some push champions that are decent in twisted treeline are Jayce, Cass, and Mordekaiser.
Lissandra seems like a pick or ban champion because you can't gank her and her cc/damage/push is very powerful.
