Okay, downbeat should be set on 00:00:539; 00:37:860; 00:53:396; (picked from Insane modding, that's metronome reset).
So we will have three red lines, placed on that points.
Your kiai times are also not on downbeats due to that

00:12:681 (2,3) - these sliders confused me. i think that they have to be placed 1/2 earlier (so their start will land on heavier beats and do not confuse the player). at least that would sound better placed 1/2 earlier

No actual errors in mapping, this is done well. Only one thing confused me at first — the same SV as in Hard, but it's mapped in different way, so this is okay (at least distance snap is lower here).

01:24:825 (1) - i may say that this slider is unwanted here. too many sliders in a row. i know one easy-player, and spinning is very hard thing for her, so consider to removing at least this spinner. this is really the hardest thing in the diff.
