Rig the elevator.
The elevator gets jammed and is stuck in an odd position. You're only able to see about 1/4th of the floor's elevator door. It gets pried open by some outsiders and they attempt to pull you out. The elevator's cable snaps and the elevator plummets while half your body is through the door.

That's highly unlikely, though? There are extra cables to catch the elevator, so something like that happening is unlikely.

That elevator story (minus the rigging part) actually happened at my University. That's where I got it from. I heard that everyone was scared shitless so no one dared to use the elevators for the rest of the semester.
