Ok, I am fairly new to mapping and modding but I still would like to think I could help you :3
Some simple rules:
I only will mod standard
Anything under 6 minutes I will try to mod
I do have the right to reject something if I don't like it
I will not mod troll maps
I will mod pretty much any difficulty, I want to become well rounded :3
Some things to keep in mind:
I am, as I said, fairly new to mapping, so if you already have a bunch of mods or are an experienced mapper don't expect the most helpful mod ever from me, but I will try to help to the best of my ability ;D
I would prefer M4M but I am fine with doing NM

As for format, I don't really care, just make sure it includes the link to the song and whether it is NM or M4M
