The gap between hard and insane seems pretty big,

you might want to change the settings by using more precise decimal settings to narrow that gap.

And you should add an orange combo color to fit with the oranges/red scenes, instead of the green combo ^^

Came as requested.

General:

-You have Widescreen Support on in Normal/Hard/Insane, but not in Demise. Since you don't use an Storyboard, you should disable it on every diff.

Hard/Insane/Demise:

-During Kiai you have several whistles around there. You should remove them (honestly, I don't think they fit).
-Insane was (for me) your best one.

...that's all (too few :( ). Quite nice map you got there! :)
