1. The Artist name, well it's not major error probably and it's just my suggestion, but give a space from each other. Now is '花澤香菜&伊藤靜&豊崎愛生', you can seperate it to '花澤香菜 & 伊藤靜 & 豊崎愛生'
2. There's Title Unicode error...
   ミンナノナマエヲイレテクダサイ(TV Size) if romanized it'll become 'Minna no Namae wo Irete Kudasai(TV Size)', not 'Minna no Namee wo Irete Kudasai(TV Size)'
3. Remove 'ディーふらぐ!' from tags since it's already on source, and change 'd-frag!' to it's full name, 'D-Fragment!'. You can add the main character as additional
4. I'll give you a video for this, i'll send it via PM, check it soon

Once the issues have been resolved, you'll need to ask BATs to bubble and requalify the mapset once more. However, it is recommended that you get further modding to polish the set further before going for qualification. Good luck.
