If i had to take a wild guess, i would say this originated in S4 League, and not DJMAX. Pentavision just happens to be the creator of both games.

the patterns are perfectly placed!

Insane:
00:34:663 - new combo here in case of two beats further.

Hard:
01:27:163 - There are two of these cross patterns that may be hard to read. You can put a new combo in every first beat in the middle of this cross to make it more readable > (screenshot below my post is at 01:28:317)

Normal:
All good.

Easy:
Fine.

I usually set new combo on the first beat like you play the instrument.
