Because when people saw a break, it usually implies "you have 5-15s time to reposition your mouse and have a rest..." (according to the mapping guidelines) a too short break may just give players next note when they're not ready.

Combo colors: Tag mode players are going to hate you. :P
Sometimes the hit sound volume raises up a bit too high compared to the song's crecendos. Give it another listen and see if anything sticks out bad to you.

The slider speed changes were getting disorienting after a bit. Sometimes it would make sense with the music, other times, it didn't feel like a good spot to slow down at. Meh, looks like everyone else loves them though.

You have some timing sections hanging around inside your sliders, which makes the sliderslide sound all weird. I suggest adding blank normal-sliderslide.wav and soft-sliderslide.wav to your beatmap to mute them. I think it would sound better.

1 minute of the song isn't mapped. Unless you're expecting to map that part, crop it with Audacity or something similar. (I can understand you kept it there for effect, but nobody is going to stay on the ranking screen for 1 minute, and the song didn't "end."
Grey isn't a recommended combo color due to when waiting for turns in co-op your notes are grey. Perhaps they could be a dark greyish red or something like that? (Oh and, white notes aren't as bad.)
It's difficult to pin down when slow sliders should be used, but it's mostly for effect. The slow sliders here are the exact same length as other sliders so you do not expect any of them. They slipped me up a few times on this map and you might've overused them.
I don't like the difficulty names, they seem ambiguous.

Slider ticks can barely be heard on certain sections.
Considering the enormous ammount of blending slider ticks your better off creating your custom slider tick that doesnt blend or changing that color.

1.) 2 is a pretty slider, but unfortunately it's blocking itself. Please try not to make them overlap. Some people actually went left, while other people actually thought the tail was the head (me). Please do something about this! 1 is also interfering.
2.) 3 is almost offscreen, do something about it

This isn't a case of preference anymore; these things are actually broken.
