The spread between the two highest difficulties is a bit high? I don't think most people mind, but I would ask some QAT just to be sure. On what grounds though xP. Spread looks fine to me. In terms of rhythm, Insane and Extra are almost identical in many parts. The main transition between Insane and Extra is the increased spacing of 1/4 circles, increase of consecutive spaced circles (from 5 to 9, basically 1/1 to 2/1). Insane still uses a healthy amount of 1/4's as you can see by the density (903 objects) in Insane to Extra's 1018.
