The same two sets of three notes just don't feel right all spaced out. But of course you can get away with that sort of stuff in Insane. The only problem I had was that [Hard] is just a bit too hard to be hard. It's like an easy Insane.
Sorry, that's intentional (it was in normal too) and i don't think a player can get confused when playing that, it's easy to guess

I can't really say much about any of your Insanes, they're just too hard for me to feel o.o so I can only give you information on normal and hard.

00:19:00 (1,2,3) - All of these, uhh, triples... Regardless if they're cool, they may be frustrating for newbies and intermediate players. It's an unexpected spacing change, and I don't see it necessary either. Surely they can be doable once you play the map again, but still...
