There are unoffical sites for Osu! that I pick from. So yes I do use custom skins and I think alot of other people do aswell. There is a forum for that too
Usually just edit color or cursor trail from unofficial skin, but for the last couple of months I made the interface myself and for the gameplay I just took it from my most favourite skin.
I use 2 skins, which I would like to learn to combine them into one in the future.

Making your own skin seems difficult. I've tried it once with ms paint and it did NOT turn out so nicely.
