1) If you want more than one thing to be done, such as both an avatar and signature for example, you should use 2 forms. One for the avatar and one for the signature.
2) They may tell you that you need to supply your own image link as leaving absolutely everything up to the artist may be to difficult as they don't know what you'd like. If you want something anime related, you can search for 'anime girl renders' on google images and find one you like and put the link in your form.
3) As for the border, if you look on the first page, all of them are listed along with an image of what they will look like so you can choose one.
If you click the edit button near your post, you can update your post if you find your own image you'd like them to use and/or make another form for your banner. Your post should look something like CoffeeLatte-'s request one post up from yours if you're requesting more than one thing. They may be able to understand what you're asking for, but I'm attempting to help make things easier for them. Let me know if anything is unclear.
