Your offset is early in quite a few places. Ask someone who's good at timing to help, as it often switches between early and just right.
01:03:793 (4) - This could probably have a more interesting hitsound, you could probably use that hitsound on the tail of 01:25:221 (1) -
In hard, there's a lot of tricky patterns and high spacing. You need an advanced diff for a proper spread between normal and hard . I could say nerf hard or buff normal, but I don't think it's possible to still have a good spread from hard -> insane while nerfing it. Adding an advanced diff is the best option.

04:22:995 (2) - I cannot do this accelerating stream at all would suggest to keep one speed since that's really hard for insane
04:33:132 (1) - Can start using 1/3 streams here instead of sliders

04:23:902 (1,2,3,4) - This one's also hard to read, overlaps + distance spacing shouldn't be used together
