Maplestory site. Banned because the owner didn't like me. Permabanned 3 times. I can still make a new account and re-join, because he sucks at IP banning.
VMK. 3 times before game closing. (from 2 hours to 1 week.)
Ventrilo. 3 times (not including joke-bans). Permabanned.
osu IRC. !killed 3 times for 1 day each. Currently in !kill status, awating results.
I kept a full list somewhere, but it's on a different site (probably from one I got banned from).

the only place I ever got banned was a private tracker (I downloaded ones and never reseeded it) other than that maybe some accounts deleted due to inactivity

Well, I got banned in here and I got banned in an Umineko forum. Too many newfags asking the same damn question, so I trolled the fuck out of them (and I was admin too lol).
Been banned from some gay ace attorney forum because there were so many newfags I ended up just trolling them all.
