You should fill these gaps in for this difficulty. This also applies if all you have is an LN in between the beats, such as in 00:40:785 (40785|5,41142|3) (personally i think trills would be great here). I'll stop pointing these out to make it easier for both of us.

Finger balance
This diff generally has alot of notes at columns 1, 2, and 6, 7. From what I can see, alot of it seems to come from trying to make your patterns look symmetrical. Thought that's nice looking, unless there's a need to put emphasis on particular sounds, try not to put too many notes on the same columns.
You have small anchors like 00:29:714 (29714|0,29892|0,30071|0,30249|0) - 01:35:428 (95428|0,95428|6,95606|6,95606|0,95785|0,95785|6,95963|0) - , turns (?) like 00:33:464 (33464|0,33464|6,33553|5,33553|1,33642|0,33642|6) - 01:08:642 (68642|0,68642|6,68732|1,68732|5,68821|0,68821|6) - , and other stacks like 00:37:303 (37303|5,37303|1,37482|1,37482|5,37839|1,38017|5,38017|1,38196|1,38196|5) - . Most of the stacks in this difficulty are at 2, and 6, which isn't really good for finger balance and can tire people out needlessly on certain fingers. Additionally, alot of the smaller stacks are placed on columns 1 and 7, which attacks the weakest fingers of a player (ring fingers for space users, ring and pinkie for pinkie users.) I highly recommend moving some of your notes to columns 3, 4, and 5.
Hmmm... that worries me... Remapped those parts a little bit, except 00:37:303 (37303|5,37303|1,37482|1,37482|5,37839|1,38017|5,38017|1,38196|1,38196|5)- they're following pitch, and 01:08:642 (68642|0,68642|6,68732|1,68732|5,68821|0,68821|6) - these should be fine for 6 stars players.
I was planning not to fix them to emphasize the word Torture.
Do note that you use a lot of patterns double stairs like [13] [24] [35] [46] [57] which can be quite uncomfortable to hit if overused. Changed a few.
00:29:892 (29892|6) - I recommend moving this to 4 for better readability.Remapped that part
00:30:249 (30249|0,30339|1) - It would be nice to swap the columns of these notes. they are for bell pitch
00:48:642 - ~ 00:49:714 - How does this pattern look? Cool!

01:27:213 (87213|1,87392|1,87571|1,87749|1,87928|1) - You might want to get rid of this anchor. Simply moving 01:27:481 (87481|4,87481|3,87571|1,87571|2,87660|4,87660|5) - 1 column right will remove it. Anchored on purpose for pitch, but alright
01:42:169 (102169|2,102303|2,102481|2,102660|2,102838|2,103017|2,103195|2,103374|2) - Now this anchor is really long. And the hand balance doesn't seem all that great either. I do have a suggestion for this section but it does break your patterning concept. Suggestion starts from 01:41:856 - .Anchored on purpose for pitch. Changed them a little bit though. (About hand balance, it looks fine to me though (Right thumb spacebar player :p))
01:46:409 (106409|1,106588|1) - Please move this to 3 and 01:46:766 (106766|2,106945|2,106945|1) - 1 column left. OK
01:47:481 (107481|5,107481|4) - And this 1 column right. OK
01:48:641 - ~ 01:57:213 - I would recommend moving some notes to column 4 and toning down on the trills a bit. This section is too dense for me to fix anchors without repatterning so no suggestions Sorry. ....Fun and unique mapset.

Remapped that to index jacks and mid+index trills. Should play better now.

- Regarding the diff spread. I'm not really a fan of the gaps between Hyper to Another and Another to Insane cause you skipped the 3* and 5* range which are pretty big. Hyper to Another is pretty reasonable imo since Hyper's SR is sub 3*. For Another to Insane that would mean nerfing the last patterns which will take time to come up with something exciting... (rip 6* tho)
On top of that, (Torture on left, Pain on right), you have places where torture is less dense than pain https://osu.ppy.sh/ss/6969035.
That's one common gimmick in insane BMS charts so I'll keep that for now until another BN calls out that issue.
In addition, some of the rhythms you used aren't really given an introduction in the lower diffs like with this https://osu.ppy.sh/ss/6969042 cause Pain didn't use any 1/4 or anything and so you basically went from 1/1 -> 1/8 jump. Fixed that one.

00:19:982 (19982|5,19982|1) - Imo, the kicks kind of die out at 00:19:982 - so it could be cool to just remove one note from this jump and move it to 4.

Since these sounds blend really closely into another, I think it'd be a good idea to try and represent that "close" kind of sound by making them all rolls without empty columns between them.
