I had a hard time finding a fitting background with Miku, I then stumbled upon this picture and went with it cause I felt like it was what I was looking for.

Spinnerstarts are actually intended to start 1/16 later so the hitsounds from the previous spinnerends would land exactly on the vocal beats. Not sure what exactly is inconsistent about this, i did this specifically at positions where the vocal is at its climax.

First time this vocal pitch appears at 02:38:150 - it has more prominence, the second one doesn't have the special feeling to it anymore since it's the same pitch again. I will leave it to Fuccho to decide whether he wants the same spinner pattern here as you mentioned, if possible I'd like to keep it there though.

(If Spaghetti deems it to be a necessary fix, you can change it for me)

I don't think there's much change in emphasis from the first to the second here 02:38:150 (1,1,1,1) so it shouldn't hurt changing 02:40:057 (1,1) to match 02:38:150 (1,1) !

02:02:811 - this beat is kind of weird because vocals aren't really 1/2, however, guitar fits for the 1/2 beat so it isn't completely wrong. thing is that it might be confusing because you were following voice on similar instances in the music, and players might be expecting this note to be an irregular rhythm. if i were you, i would snap this beat as a sliderend instead or simply move the note to 02:02:768 -

last point isn't unrankable, because as i said, guitar is correctly snapped, but i believe it will improve playability if you consider that point. Joey's diff snapping should be fixed, though

Can you reduce the SB load at around 01:58:616 - and 04:06:751 - ? It's being unnecessarily high at these two points.
I think you can make the blur.jpg (or dark or whatever elements you used at these points) fade out faster to avoid too many SB elements stacking together at the same time.

also on all diffs:

02:02:811 - this beat is kind of weird because vocals aren't really 1/2, however, guitar fits for the 1/2 beat so it isn't completely wrong. thing is that it might be confusing because you were following voice on similar instances in the music, and players might be expecting this note to be an irregular rhythm. if i were you, i would snap this beat as a sliderend instead or simply move the note to 02:02:768 -

last point isn't unrankable, because as i said, guitar is correctly snapped, but i believe it will improve playability if you consider that point. Joey's diff snapping should be fixed, though
I'll wait for Joey to fix his and get it updated.
The second mention is something I won't be fixing since as you say, it works either way and would be too much of a hassle considering the outcome.

I also doubt if the rhythm 03:13:107 (3,4) - in toykii's Insane and Pho's Hard is rankable.
as there're 1/3 beats from 03:13:107 - to 03:13:616 - , that means there's no beat on the 1/2 red ticks 03:13:234 - and 03:13:489 -

Also, 03:12:599 (1,2) - in Ruthless are qustionable as I believe 03:12:853 (2) - should end on 1/4 blue tick 03:13:044 - or red tick 03:12:980 - and 03:12:599 (1) - should better be the same as 2 to make the rhythm easier to grab since there isn't any map-worthy beat at 1's end
I don't entirely agree. I think in this case (toykii's and Pho's) it's tasteful undermapping done throughout the entire to make the reading easier on the player (since they're easier difficulties). 1/3 might be more technically correct but I feel like the current works as well.

And about my part: I don't see the problem. There's not really any sound on any ticks after the white, so that's where the clickable sounds are. Making them 2/3s was a choice I made later, cause I felt like it'd make the next 1/3 burst more intuitive to play. Not something I feel like changing, and definitely not something unrankable.

Still only changing [ Joey ]'s unless toybot and/or Pho want to change theirs.

on pho's diff it seems okay because easier rhythm and still fits with vocal pattern mapped before + it's not as noticeable due to silenced sliderends. still think 2/3 sliders would most likely work just fine (but I didn't look at the whole diff )

on the insane it's doesn't really work though, even less so with the pretty audible hitsounds on the sliderneds creating a totally unfitting rhythm. http://i.imgur.com/vNqzbMi.jpg would still be easy rhythm wise and actually match the song.

from a quick look there seem to be more of these things in this part like 03:15:141 (2,3) -
