I am using http://flags.blogpotato.de/zip/small/world.zip as my source - it is licensed under Creative Commons BY-SA, so you should have no problems using these (if you like them, anyways, lol).
Anyways, here are the missing flags (404ed flags) from http://osu.ppy.sh/p/countryranking?p=countryranking (and profiles from users in those countries). I manually edited a 1-px black bar around the image, whilst keeping it 18x12 like the others. The current images have this black-bar as well, so I don't want them to be out of place.:
