At tom94's request, I've made this thread. The issue is that when you hit a (repeat) slider with a button, then you quickly switch to the other button to keep holding it, the slider doesn't register the new button to be holding it and breaks. At least that's what I think happens.
This mostly happens during fast streams that end in repeat sliders, when you overstream by 1 note.
