Sudden death. I keep forgetting to turn it on, so i always after losing a combo go to menu and retry the song.

it depends on the beatmap, but lately i prefer hidden over the other mods

I agree that Easy sometimes often makes a map ridiculously hard to read, but you can't tell me that HardRock makes all maps easier. 
If you use HR only for Normal maps, it's quite obvious you think that way, but have you ever tried HR in a hard map with Overall 7? You get Overall 9 and it's really hard to get a FC that way, but i find it really fun and challenging in some maps.

Oh, and try to pass this map with Easy mod without dying (since you get 3 lives).

Some hard maps are easier if I play with HR. Like, Lemon Tree and Umbrella.
I use SuddenDeath almost always, then i don't have to pause when i break combo (but i still have to when i hit 100 D:). I don't like DoubleTime, cause many songs with it sounds like puking monkey. Other mods - neutral.

Hidden+Hardrock most of the time. Or just Hardrock.
I like to play with Flashlight too, but it takes a lot of time to memorize the map and I don't have enough time for this nowadays =\
Simple hidden is just not interesting for me and not really challenging

used to be HD DT, but HR HD nowadays.

1. HD (alot recently.. i'd study maps on auto/spun out first (~2 plays), point out difficult areas, then attempt for the S. pretty sure others do this and this is the way to go, rofl)
