This guy was around for a while, I actually saw him too once, back when chatroulette had only like 300users and everybody would pretty much seen every body else after being on the site for 2-3hours.

Besides, you can't pinpoint a single video. I've seen quite a multitude of self mutilated man-genitals here on these tubes. And none of them I saw intentionally. Just be wary of what kinds of sites you browse or man up and learn to stomach it. People only get more sick.
