Need help finding an old skin

I've been trying to find this skin for years now and have had no luck. It's a pretty simple skin but I just can't find the right one.

The skin is pretty simple so I guess your best bet would be to recreate it. The only thing you'd need to change is the hitcircleoverlay and the default numbers to make the gameplay feel like it used to.
