I turned mine off a while ago, trying to figure out what it did. Since it didn't make a noticeable difference, I just left it off. At least now I know what it does.

The amount of difference it makes really depends on the skin you're using. For example, with my skin:

Judgement Line enabled:

You only really need to enable it if your button skins have a gap at the top of the ".png" and not even a red line like Yaymii's example. So there would be just the white line, and then disabled there would be nothing but a gap. You still have to hit the note at the position of the judgement line whether it's on or not. It's your "hit it now" line, which you can optionally skin into your buttons for yourself and turn off in options.
