Which difficulty should I start?

Let's say I'm new to making osu! beatmaps. I read most of the tutorials here, so I guess I'm all set. The next logical step would be making a map at the lowest difficulty (Easy), but I feel it doesn't give much of a challenge. Should I go on and make an Easy map first or make a higher difficulty straight away?

And an unrelated question: does the editor permit 4-key osu!mania maps at Hard-Insane diff? The editor seems to default to 7-key.

First question: It would be easier to start with the hardest diff first and work your way down. It is easier to delete objects then to add.
Second question: Sorry don't know much of osu!mania >.<

I am going disagree with Weez about which difficulty to map first. Every difficulty should not be differentiated just by adding notes. The fact that note density and, slider velocity change between every diff means that every diff will turn out unique to an extent. Personally, I just say map whatever you want first, whatever comes to your mind/interest, since I feel the biggest downfall in mapping is lack of motivation leading to lack of creativity.

As for osu!mania, you can have as many or few columns as you want for any diff. Just go to song setup -> difficulty and select the number of columns on the second row.

Yeah since there are so many differences between the settings for each difficulty in standard, you shouldn't try to make 1 diff and just add/remove notes for the other difficulties. Even if you could manage to make a pattern that somehow worked in all difficulties, it would likely end up uninteresting and awkward to play in the difficulties created after the first one you made.

As for mania, I want to say that it is highly suggested to not try to do things the way standard maps are converted. 4k is perfectly acceptable at the Hard-Insane level, just as 7k is perfectly acceptable at the Easy level.

Actually, it's generally a good thing to make each diff as familiar as possible. I like to start with [Insane], because it lets me explore as much of the song as I want, thus defining an upper difficulty limit. It also lets me see how far I can go with specific types of patterns. Plus, it's much more relieving to get the hardest diff out of the way first, so I work my way downwards and have an easier time towards the end of the mapset.
