Problem details: Usually the system has that time when HP fills up and note starts to show up, but when in editor the note shows up too fast (first frames), then it could end up like this.
Edit 1: The actual gameplay is normal.

add some blank space to the start of your mp3 file, then shuffle along the notes. This is a somewhat known issue with having a very low offset
