Hi there. Sorry for the late delay. You posted a request on my modding queue: t/336458/start=120 and thanks for modding my map (although your mod was unfortunately based on an older version of the beatmap).
Note that whatever I say is my own opinion so you do not have to follow everything I say. Changes are separated into boxes.
Final thoughts: I am not very good at modding extra maps, as I really do not know what fits well. However, I can say that the rhythm is very well done, but where you place the notes is a different story; lot of the time you do not make use of the whole playfield, which means you compress areas together, intersect previous notes and hence looks messy.
Remember that you have to have a full set of difficulties if you want this map ranked.
Good luck and I apologize for the delay, and the mod which may not be up to your standard. Get some more test plays if possible.

General
1) Your difficulties were challenging to say the least. However, if you want to rank this map, you will have to make the easy, normal, hard spread as well.
2) 00:01:565 As the song starts, you can map a spinner here which ends at 00:03:742. Since the music is gradually getting louder at the start, I would recommend these following timing points.
- 00:01:565 5% volume
- 00:01:876 15% volume
- 00:02:187 25% volume
- 00:02:498 35% volume
- 00:02:809 45% volume
- 00:03:120 55% volume
- 00:03:431 65% volume
- 00:03:742 75% volume
3) The hitsound volume is too loud at 100%. I would recommend lowering the hitsound volume to around 65-80%
4) Add some audio lead in. Perhaps between 1500-2000 ms. You can add this by opening the .osu file in notepad.

Extra
1) 00:09:648 The stream overlaps this slider (00:09:027). Try and avoid overlaps like these because they are messy, therefore make use of the whole playfield. Perhaps as a rule, try to not overlap objects if they are around 0 - 3 seconds apart.
Right now, you can move it here:
2) Probably applies to ultra as well. I think that you may be abusing the use of hitfinish. Ask someone else who is modding to confirm, because I think that some places where you add hitfinish can be replaced with hitwhistles. Examples include: 00:13:690, 00:13:845, 00:14:933 and 00:15:089 etc. Try find some custom hitsounds to spice up the map.
3) 00:14:001 I think here is where you add a hitfinish. This is an example so there are many more areas.
4) 00:20:685 Perhaps try a star pattern:
5) 00:22:783 I don't think there should be a triplet there. However, the problem is that once you remove this, then 00:21:928 to 00:23:483 are all single notes, which may be boring. Then again this is just me.
6) 00:25:192 to 00:26:125 make all of these sliders the same shape.
Other instances where this occurs: 00:24:571 and 00:24:882, 00:28:923 and 00:29:545 and 00:30:166.
7) 00:34:519 If the distance between the notes in the triplet are very small, I would rather see them stacked because it looks better. Alternatively, you can increase the distance between the triplet notes and that will look better.

Ultra
1) I think that ultra should have the OD raised to 9, since it should reflect a really really hard difficulty.
2) 00:12:757 Perhaps go with a star pattern, the sixth note can be stacked underneath one of the notes.
3) Lot of overlapping results in the map not looking that good.
4) 00:47:576 I think here there should be a triple.
5) 01:01:565 (every 2nd and 4th white tick) add a clap sound: try this hitsound since you are using the drum hitsound set.
6) 01:11:203 to 01:11:669, 01:12:446 to 01:12:913 etc. Try this hitsound (seems to fit): http://puu.sh/jTzCV/d91b111bd8.wav
