It's weird, because TH11 and TH12 don't exhibit this behavior on mine.
If that doesn't work or you're already using it, try downloading th_pivot http://bygzam.seesaa.net/article/123123010.html. It's intended to be used by people with those crazy pivot monitors (or whatever), but I found it also let me play fullscreen without stretching. Just extract the .dll and .exe into your th folder and run th_pivot_custom.exe. I don't remember what all the options are, but you can randomly select stuff until you find one you like.
None of the options there somehow pertain to fullscreen.Okay so here's what I did: I just put the .exe and .dll inside the same folder as my game (...I'm assuming this would work with any of the games right), configured stuff inside the .exe and ran the actual th##.exe . EDIT: So I did something stupid today. It involves computer stuff; to summarize it was an attempt to make my monitor limit its display to 4:3. Eventually I ended up almost permanently sealing the primary video output to a non-existent output, thus meaning that I was working with a monitor that isn't displaying anything at all. GG. This is the first time that Touhou has been the source of a major problem of mine... lol... (well I got it resolved now so w/e , I'm still looking for solutions to make a 4:3 fullscreen orz)

The following video... well, it kind of borders on 'oh god that's disgusting'. But the artwork of this artist is AWESOME. Everyone from Gensokyo looks so badass in this vid. Also guest appearance from pink Pedobear too.
Then this next video is a cutscene for AQUASTYLE's upcoming Dungeon game (I'm not sure if it's the same one that was discussed a few pages back). Just watch it, 'tis awesome.
Also adding to the awesome is even more 3D vids. Here's a dog fight quicky of Reimu vs. Utsuho. As usual Reimu can think up of ways to hax herself out of everything. (the end of the vid orzviously implies that it's still not done)

anyways was it me or Heartful Nekoromancer that Djpop make is kinda disappointing for me. even for a long term players like me i just cant read the rhythm that was in the song. kinda sucks though.
but other touhou songs are just my style. too much fast and nice beat spacing is really the best ones i've seen for touhou songs

2. ctrl+f at drawing the pattern (i got 97.6%) =D better accuracy = better spinning power
3. ctrl+f back before spinning . i've reduce the window size here by dragging right border as small as i can xD

Team Eintei's order of importance: 1st: Kaguya 2nd: Eirin 3rd: Reisen
You should definitely focus Kaguya because she has the least health and does the most damage. (and as a bonus you can waste Eirin's turns)
Make use of the stuns that you have. (Reimu's, Suwako's, and Yuugi's)
Don't debuff them - Buddha's stone bowl will be your bane

Okay I beat them already, I just changed my formation by a bit, as soon as kaguya will almost have no hp (estimate), put suwako back in, kill kaguya, and perma paralyze eirin.
Chen + Suwako = dead eirin lol

...I was looking for it too complicated. I never realized that it was all in my monitor's settings, actually: the Image Size was set to 'Wide'. I simply set it to 'Auto'. So everytime I fullscreen anything that's 4:3 now, it appears as 4:3 like it should, so :3
Seeing this I tried to play Touhou like a confident little kid...
...and of course I got owned. Playing in Windowed for 2 months has gotten me used to looking at really small things.
But I persisted and tried and eventually ended up 1CC'ing TH9 on Normal (for the second time, without an insane amount of luck like my first 1CC) using Marisa. I ended up with a happy 87 million points (though I must admit it could've been larger if I didn't stupid up my third round against Shiki), and was able to add another ending.txt into my collection, and tried to save the replay, when I completely forgot about the Replay glitch and the replay didn't save :\ But still, at least I got Ending No.2's script wrote down~
