I think the part in the first Kiai time is a bit too difficult (or by osu standard? well at least i think it could be easier to understand)
I think an easier solution would look like this http://puu.sh/2ljTI (keep in mind the notes are not moved and i only want to show the general look of notes for the pattern i have in mind. Also i think all the half beat holds should be 1 beat long for easier playability and it matches the song anyways.

I'm still not sure about this one >< Sorry, I retained the current pattern for now. But on another note, I moved the notes for it to be easier (Distributing the hold and single notes for each hand)

hmm, I think you need some red lines (timing points) to fix the metronome. This music is made in 3/4 (waltz) from middle part! (important change for playing Taiko)

The 2nd and 3rd offset are unecessary, just leave the first one and change it to - BPM: 189 Offset: 780 (+5ms) with 3/4 signature
The slider reverser arrow presents a whitle one on combocolor 4 (the green one). Which we can tell the reverse isn't evident, and caused a problem about doubtable sight-reading. So...You may need a sightly brighter color for combo 4, to make sure it showed a black arrow properly. e.g. Combo4 : 191,255,191
1. The position of background image is sightly(1 pixel) off, suspecting there's a bug which caused such a nazi problem on every mapset. w/e please change the code manually in all .osu files!

I try to follow your mapping logic and I guess you used 2 circle in one timing point is follow the bass track, here have the bass sound too, but you only used 1 circle here. that's why I guess you miss the circle. Aah. I was following the synth here (while the middle is reserved for the bass). I didn't add a circle here because I hear no synth sound.
