You can be focused on fighting some mobs here and there or you just were running without attention to the ground and suddenly fall in a trap for example. In conclusion: Stupid deaths are easily avoidable if you pay attention when you are in a situation where it's supposed to be dangerous. For bugged deaths like a Nether portal spawning above lava and not making a safe zone (happened to me once) or falling into the void, your items are already lost so /back wouldn't help.
EDIT: By the way, is it possible to name outposts so we can do stuff like /town outpost <name> instead of using a number? Numbers are kinda annoying because they also change when old outposts are removed.
If we can, can CURRENT outposts be renamed?
