Ok, after confirming with Peppy about the non-constant BPM, here's my somewhat confusing timing panel.

00:01:20 4/4 175.01BPM
02:53:99 3/4 244.17BPM
02:56:82 3/4 243.28BPM
03:02:77 3/4 233.52BPM
03:08:82 3/4 235.36BPM
03:15:05 3/4 233.61BPM
03:21:04 3/4 229.77BPM
03:58:64 3/4 226.56BPM

You might want to tweak those numbers a little and get another mod (like Echo) to check them.

the offset for insane is 12, but it should be 1,200 like the other difficulties

Ok, further analysis shows that you actually need to resnap/recalc your notes and sliders. I'd suggest changing your beat snap divisor to 1/2 before doing this.

Basically most of the beats line up with the white or red ticks.
