How to replicate:
- in the osu!mania mode, use mods ScoreV2 and DT or NC,
- pick a map that ends only with a long note (or multiple long notes),
- miss the ending of the last note(s), whether by not hitting the note at all or by not releasing past the end,
- die instantly.
I consider this to be a bug because taking out either of the score and speed mods doesn't make you fail as described. It has nothing to do with the SD mod.

I tried the difficulty mods. I still die with EZ (3 lives gone) but survive on HR---an irony.
