I play ctb mouse-only so this is valid to me.
Never really thought about it. My free hand is used to scratch idle itches and stir food and random tasks.

On the z/x keys, even if I have no intention of using them.
When I mouse-only I either apply my idle hand (in my case, the left hand) on my right arm/wrist/hand so I can get better control and not overclick too much or go out of placement, especially on streams where my arm tends to shake a little whilst clicking.
When I'm fully mouse ages ago, I tend to put my fingers on "WASD" key even if didnt do anything there
On q/e, I feel weird if my hand isn't on the left part of the keyboard regardless of use.

My Left hand is usually on my Right Lap, leaning slightly to the right for some reason so I have better control with my Right hand.

I mean.. uhh... I use the z and x keys as a secondary option to the mouse buttons. It is much like playing taiko on the PSP, where a dominate hand will do single beats while tricky rhythm requires separation.

On WASD, and well i set my left and right keys to W & E since i play alot of fps games so my hands are usually there 
and using the keyboard to hit lots of fast beats is really convenient

my left palm is usually resting on my keyboard's cushion part while my right hand does all the work
My left hand tends hold down the mouse pad which often slides around a lot when I spin.
My right hand does all the work. My left hand Hold my chest.
Right - Mouse Move and Click
Left - left keyboard for SPIN (hold), STREAM (zx), Taiko (xv), ESCAPE.

Hmm surprisingly most people seem to leave it on the keyboard, i myself leave it on my desk while leaning forward a bit.
I wonder where taNa leaves it? In his videos you can't even see his left hand.

WASD is common placing for my right hand whenever it's idle...
It's just feels natural, I guess years of FPSs does that :/

can't really control my spin when clicking + spinning lol

On my desk. And if is a very relaxing map prob chin resting on left hand.
Even though it is childish... my hand seems to always end up on my crotch 99% of the time... not good...
