Timing seemed almost spot on to me. Try dropping offset to 300ms.

Easy:
There are those two sliders I mentioned in IRC that were a quarter beat late. Continuing from there...
00:24:73 (5,1) - huge jump for easy D=
00:30:11 (1) - quarter early
00:34:54 (1) - Bring this in a beat early to start at 34:28 and add one repeat so it ends at the same time
00:45:76 (5) - It's a quarter late.
00:47:49 (8) - It's a quarter late. Bad *newspaper*
00:51:33 (1) - QUARTER LATE
00:53:86 (4) - QUARTER LATE - I wonder if it's my fault for changing the timing =x

00:30:10 (1) - quarter late, also damn hard to hit because of the spinner. I recommend deleting this note and letting the spinner finish at 30:03
00:34:54 (1) - these stacks are kind of lazy imo. Take the dog for a walk; let the notes wander around a bit.

00:45:76 (7) - quarter late

00:45:76 (6) - starts a quarter late, so you'll have to resnap and lengthen it a bit

Normal:
00:11:31 (6) - quarter beat too long
00:30:43 (1) - Being to the right a bit is just kind of ehhhhhhhhhh I'd say shift it left to line up with the vertical axis line but that's me being a stickler for cleanliness
00:43:17 (3) - quarter beat too long
00:51:67 (3) - quarter beat too long
00:54:19 (7) - this too

Hard:
Set beat snap divisor to 1/2 and resnap all notes - there were some snapped to 1/8's and I do not know why. Then go through and resnap notes where appropriate (eg 00:24:72)
00:38:39 (1,2,3,4,5,6,7,8) - This here's a nasty clusterfuck in-game - the score explosions in most skins will clutter visibility. Sliders are short enough to avoid damaging a combo, but it'd be cleaner if you used less overlap. Let the sliders spread out some.

Bloody Insane:
it's too damn similar to Hard. I like the doubletaps you have going on, but it needs to be more different than that imo. Maybe increase overall difficulty, widen spacing, turn a few sliders into hitcircle patterns and vice versa.
Also I don't like this 01:02:65 (1,1,1,1). Maybe they need finishers, maybe it should just be two spinners.
btw those faux-repeating sliders at the start of Hard/BI? Clever.

Bloody Insane:
00:28:97 (1-4)
00:29:50 (1-4) I think you can map these differently the double clicks really just don't seems to work well imo

00:53:13(4) move the note or the slider, i suggest moving the slider path so its spaced right

I would go beatspacing nazi on this mode but I already know the beat and you snapped notes to a logical timing so meh.

Hard:

00:29:23(2) move it down a tad so it's evenly spaced
00:53:13(4) either move the note or the slider before so it's not overlapping

Normal:

00:53:22(4) again with the slider overlap and move it back 1/4th of a beat. infact make the slider beforehand end a 1/4th beat earlier too.


Easy:

00:12:57(1) have it start 1/4th of a beat earlier and last 1/4th a beat longer


Overall:

I suggest double checking your map to make sure spacing is consistent, although parts when you have a lot of notes stacked like 00:34:48-00:38:19 I generally just follow the beat so if the spacing is a little off to make a pretty pattern is fine since its not very noticeable.

Pick a distance per beat and space the other notes accordingly, 1/2 notes will be half the length of the distance from one beat to the next, and 1/4th blah blah blah you know this.

Also i see a lot of notes snapped to 1/8th notes, i suck at timing so don't take my word on it but your BPM/offset may be off.

90:13:38 - The distance between (2) and (3) should be the same as the distance between (1) and (2).
00:33:91 - I don't think (2) is close enough to (1).
00:42:81 - (1) should be closer to the end of the previous slider.
00:46:78 - (7) should be closer to the end of the previous slider.
00:55:52 - (1) and (2) should be closer together.

Normal
00:13:27 - (4) and (5) should be closer to (3).
00:23:26 - (1) should be closer to the end of the previous slider.
00:43:10 - (2) and (3) need to be closer to (1).
00:44:63 - (4) and (5) need to be closer to the end of the previous slider, and further from (6).
00:52:94 - The end of (3) doesn't snap to any note when set to the 1/2 divisor.
00:55:82 - Make (10) and (11) closer to (9)

Hard
00:06:42 - I don't think the slider at (9) is visible enough and I reckon it should be replaced with two hit circles.
00:10:51 - The slider ends a little early.
00:14:78 - Ditto.
00:18:95 - The slider ends a little late.
00:44:42 - Ditto.

I've noticed that I've seen a lot of these patterns before... It'd be good if you could change the pattern rather than just adding a few notes in here and there for each difficulty, but then it's not necessary.

00:15:00-00:17:00 - I don't like the doubled up notes here, it just doesn't feel right. The same applies at 23-24 and 25-29 and 30-32 and 49-51 and 57-68...

Just change the beats duing the Organ parts in hard and the spinners in Bloody Insane and pehaps upgrading the beats from double beats to triple beats and ill be happy

00:47:03 (8,9) - that big gap on an easy map bothers me
00:53:80 (4) - further away from both 3 and 5
00:55:53 (1) - closer to 2

normal:
00:30:44 (1,2,3,4,5) - it'd be nice if this combo was aligned so 1 is in the middle
00:33:23 (1,2,3,4,1,2,3) - shift these right by 2
00:44:64 (4,5) - move these left so 4 is right next to the 3
00:45:70 (7,8,9) - move the 7 up higher, then move 8 and 9 so that they go from the end of 7 to 10 equally spaced
00:51:28 (1) - closer to 2

Spacing can be less strict on hard/insane maps, but imo for easy/normal maps the timing should be more obvious.

You might also want to up the overall difficulty of insane by 1-2 notches, atm it's not very insane yet :P
