Most of the people who played other rhythm games sure to realize some of the game may have diffrent stage design.
take example of Tap a Jam:
it's kinda like osu!mania 4K with sideway stages.

It's kinda like 6K with sideways, with each 3 notes comes from diffrent sides
from Beat-Up, a mode in Audition Online, which kinda popular in some countries.

to get right to the point, we can make a whole set of skin.ini to configure so default skin graphics could fit the customized skin,
or add the ability to fit specific graphics for specific skins, i bet someone somewhere who want will made the graphics with their customized skin.ini coding.
take note to IIDX mania for example to know how much this fanbase can do
I bet osu! will be more popular because other players from other game with specific stage design would be more familliar with osu game mode right away, thus gonna stick and recommend osu to his/her friends.
and for the copyright issues, well, it can't be helped, fans could find a way somehow.
