My first attempt at modding so i could be completely wrong.
Insane
1.Spacing between the notes of the stream 01:29:153 (2,3,4,5) and the next slider 01:29:517 (1) should be the same (right?)
2. 01:42:426 (2), 01:42:790 (3), 01:43:153 (5) - probably ok but might look weird on 4:3 resolutions

01:33:699 (2) - too high up i'd say
01:37:881 (2,3,4,5) - personally i wouldn't put such streams in a 4 stars song but it's your map
01:40:790 (2,3,4,5) and 01:41:153 (1) same as 1.
01:43:335 (1), 01:43:881 (1), 01:44:426 (1) - kinda difficult patterns for 4 stars
02:21:517 (2,3,4,5) and 02:21:881 (1) - same as 1.
02:24:426 (2,3,4,5) and 02:24:790 (1) - ^
02:33:153 (2,3,4,5) - same as 3.
02:34:790 (2) - same as 2.
02:36:063 (2,3,4,5) and 02:36:426 (1) - same as 1.

00:44:608 to 00:56:790 - What's with these timing ticks?Might want to take those out.
01:29:153 to 01:29:426 - I like the style,just don't really like this part.Optional,but recommended.

Overall,remember streams.Around areas like 02:40:063 would work somewhat better with a stream instead.
Other than that,good job!Love the map and the flow.
Not much more to add,sorry.

Difficulty Spread
Would probably work with one more harder diff.It flows nice,and the map overall is great.I would add an extra difficulty,personally,but that's just me.

On the Insane diff, turned on HR, couldn't get past first spinner... lower HP drain?
