You should fade out the evil shadow at 02:41:989. Even though it's covered by the spinner, you can still see it when you play catch the beat and it would look bad just suddenly disappearing. By the way, here's your better quality mp3.

Easy:

02:35:934 (1,2) - SYMMETRY FAIL

Hard:

01:44:741 (3,4) - This one caught me off guard. It could totally be cool if you horizontally and vertically flipped it though.
01:51:347 (2) - Cuts through the bottom of the screen, you'll have to raise it up and adjust the rest of the pattern somehow.
03:06:760 (1) - Missing a clap at the end it seems.

I'd like to see kiai just in the last chorus... Dunno why, I think it is better just there.But whatever

Easy:
01:28:228 (1) - delete new combo?

Hard:
01:14:329 (3) - That's waay to suddenly... but I don't know how you could change that without making it ugly :I
01:27:127 (5) - I was thinkin this would be like (4) vertical flipped..
01:42:264 (1) - place it where the slider (4) started
02:41:989 (1) - make the slider 1/4 ending earlier so we can see how that cool SB effect..

Enable epilepsy warning? It looks like it needs it especially during the kiai
But will leave it up to BAT to decide

Maps follow lyrics slightly too much for my liking but they are nicely done either way~

(I meant at liike the end of the kiai time)

I don't see how one slight pulse every four beats deserves an eppilepsy warning

It's.. not.. so.. hard!

And the epilepsy warning is not necessary. If you get epilepsy from this, you'll probably already get a seizure when you blink.

This is definitely one of your better works. Hard is awesome, and I loved those stars :P
I can't for the life of me figure out why people downvoted this. Maybe because there's no Normal?
