Do you gain supporter status only if you pay per month? Or if can you also buy something from the store such as an Osu! Tablet and also gain supporter tag?

You lose your supporter tag after it expires and buying from the store does not give you supporter tag.
Also just a little tidbit to know but you get to keep some osu! supporter privileges once you lose your tag (userpage for example)
Yeah, all you have to do is pay monthly to keep the supporter tag.
As kirueggy said, you do get to keep some features. So, if you pay for 1 month, you'll still have certain perks after your tag expires. Another one is keeping your feature request stars, which you get 2 of for each month you pay. They can be used to vote for feature requests you would like to see get added to the game, so that the request will be able to be seen more often
