I fixed the kiai things you mentioned, and also tweaked the hard vocal part a bit. It's pretty hard to get it directly on-vocal though without being awkward.

Also, hard is now insane. I'm looking for a guest mapper for hard difficulty, if not I'll just throw one together.

02:55:149 - This feels like a weird spot to start kiai considering the music and the mid-spinner thing.
Think you can find a better bg? It gets really artefacty when it's stretched out so much. :/

Compare the new combo use at 01:06:316 (1) and 01:08:983 (1). Whichever way is fine, but try to keep it the same.

Think you can find a better bg? It gets really artefacty when it's stretched out so much. :/ I could look around. I found it in lazy-mode, and it looked pretty nice on 800x600.

Kiai continues to end. I thought you were supposed to turn it off, but I honestly don't remember anymore. I also thought the difference in Kiai was a bit odd, just because m980 used it only for the final chorus in one big block, whereas Rolled did for the chorus 3 times in a just slightly shorter period. The edge notes and kiai are mostly just observations and personal preferences.

I don't really care for the giant circle-high velocity style. 2.20 at 180 BPM is kinda crazy for me. Despite the high velocity, I'd say consider -1 Circle Size, I don't think it'll affect play, circles pretty big as-is.

I've currently lost my headphones, but i think the 02:23:483 (4) - could use 1 tick to the right (landing on the blue beat). Personally i would use a slider here like this http://osu.ppy.sh/ss/14253 (circles could also be used, this is just an example :P )

same goes for this 02:28:816 (3) - this note could use 1 tick to the right so it lands on a blue tick.

anyways, for now i would say trust your ears on the mod above seeing as i don't have my headphones. But to me it feels like it syncs better in gameplay and vocal.

Fixed the combos in m980's difficulty as per his request.

That slow part on insane mode.. is a pain. I'd rather have them all steadily 1/2 or 1/1 beats apart to try to eliminate confusion, even if it doesn't time with the vocals properly. Don't really want to add another slider there other than the long one.

The offset too, I don't really agree with. 5 ms can be pretty subjective, though. I think it will hurt the already seemingly early slow section with -5 more ms.

Found nothing wrong/bad for me, except for one thing, pretty much the same as what gabi said...

Insane:
02:22:816 (1,2,3,4,5) and 02:28:316 (1,2,3,4) would sound better if they were 1/4 tick forward. I guess this is because the vocals is way too prominent to be ignored.

I guessed i should say this too, because the way it is, is way too confusing too imo.

The rest is perfectly fine for me
