It's interesting, but I have the same problem. It's really annoying on for ex. on this map - http://osu.ppy.sh/b/25878
Though I think it is because of the way I am using mouse
My mouse is 600 DPI, sensitivity: windows - max, osu - x3.5 and mouse precision is enabled

It has to do with mouse acceleration.
Mouse acceleration influences the relationship between physical movement of the mouse and on-screen movement.
When the mouse is moved at a faster pace the cursor will move further than the actual distance covered.
In osu! this means that if you move your mouse up at a slower pace to reach a beat and then return to the old cursor position at a faster pace your physical mouse will be positioned slightly higher then it previously was.
This explains why your mouse position is way off from its staring position even though the cursor will only move in a small square area.

To fix this you'll have to disable it in the mouse control panel. Be warned though that if you're really used to this setting it takes some time to get back to your old playing ability.

For me, this happens because I rotate my mouse slightly when I play, usually when I move up, so effectively I'm moving more down on average, physically, causing this problem after 20 minutes of playing. At least there are breaks in [most] marathon maps to fix this. =]
